Job Title: Clinical Quality & Safety Officer (CQSO) – Oncology

Job Summary

  • The Clinical Quality & Safety Officer provides clinical-path leadership for patient safety and quality across the network. This role owns clinical governance, Infection Prevention & Control (IPC), radiation protection (ALARA), incident reporting and learning (RCA/CAPA), clinical audit, and accreditation readiness—publishing safety dashboards and coaching teams, while running daily rosters or throughput.

Key Responsibilities
Clinical Governance & Risk:

  • Maintain the Quality Manual; version-control JCI-aligned policies/SOPs.
  • Manage the clinical risk register; lead FMEA on high-risk processes (chemo, RT, contrast, invasive procedures).

Infection Prevention & Control (IPC):

  • Implement standard & transmission-based precautions; lead hand-hygiene and reprocessing/cleaning audits.
  • Monitor Healthcare-Associated Infection; coordinate outbreak response and statutory notifications.

Radiation Safety & Therapy Quality:

  • Enforce ALARA with RSO/Physicists; ensure staff dosimetry and area monitoring.
  • Verify LINAC/CT-sim QA schedules/results; apply lockout criteria and escalate deviations.

Medication/Chemotherapy Safety & Incident Learning:

  • Drive chemo safety (independent double-checks, closed-loop orders, emergency drills, LASA controls with Pharmacy).
  • Operate a Just Culture incident system; lead RCA/CAPA; issue Safety Learning Bulletins and verify effectiveness.

Audits, Accreditation & Training:

  • Run rolling internal audits; maintain evidence files; lead readiness for JCI/ISO/local regulators.
  • Publish/validate the safety dashboard; deliver mandatory training and annual competency checks.

Qualifications

  • Bachelor’s in Nursing/Pharmacy/Allied Health or Medical Physics; Master’s in Quality/Patient Safety preferred.

Experience:

  • 5–8 years in clinical quality/safety within oncology/RT/chemo or high-acuity services.
  • Proven leadership of IPC programs, RCA/CAPA, audits, and accreditation readiness.
